Access Choate's Online Pre-Interview Information Form and Application
Even if you have already completed a handwritten copy of the Pre-Interview Form (or the Preliminary Application) you must complete the Pre-Interview Form online in order to access the Online Application. The online forms are linked, so the information you enter in the Pre-Interview Form will automatically flow through to your Application.
Required Supplemental Forms
In addition to the Online Application, there are five supplemental forms which must also be submitted to the Choate office. Links to these forms are at the bottom of this page.
The required English and Mathematics Teacher Recommendations must be completed by the appropriate teacher and sent back to Choate. An optional Supplementary Recommendation which may be completed by an additional reference of your choice is also provided.
The School Report must be filled out and submitted by an administrator at the Applicant's current school. The Graded Essay Cover Sheet must be completed and returned with a photocopy of an original graded English or history essay.
