Application Materials
The Choate admission process begins with the Pre-Interview Form, which must be submitted at least two weeks prior to your interview. The next step is to complete your Application.
You can access the Choate Pre-Interview and Application materials in two ways:
- Applicants are encouraged to complete the Pre-Interview Information Form and Application online. The Pre-Interview Form should be completed and returned at least two weeks prior to your interview and tour.
- You can also download the Pre-Interview Form and the Application from a PDF file and submit the paper version via mail.
Application Instructions (PDF) are also available. In order for an application to be reviewed and evaluated by the Admission Committee, it must be considered complete. A completed Application consists of the following:
Submitted by the Applicant on paper or online.
- Applicant and Family Information (PDF)
- Applicant Interests and Personal Statement (PDF)
- Parent/Guardian Statements (PDF)
- Graded Essay — A photocopy of an original graded English or history essay. (PDF)
Submitted on the Applicant’s behalf by school officials or others.
- School Report (PDF) — including transcripts and current grades.
- English Teacher Recommendation (PDF)
- Mathematics Teacher Recommendation (PDF)
- Supplementary Recommendation (PDF) — optional
Application Fee: $50 for U.S. applicants; $100 for international applicants.
