International Students (non-US citizens)
If an international student is currently studying in the United States on an F-1 visa, we would need to discuss with the student the possible "transfer" of his/her visa to our summer program through SEVIS. By NOT doing so, the student's visa status might be jeopardized. If you have not done so already, be sure to contact our office via e-mail or phone (203.697.2365) so we may discuss the details. To All Accepted Students and their Families
Upon acceptance into one of our academic summer programs, a student is mailed an acceptance letter and Summer Enrollment Agreement (contract). All information posted here is either required to be returned to the Summer Programs Office as soon as possible (but well in advance of the student's arrival), provided for your information, or to be ordered from and paid directly to a vendor for outside services. If a family needs to quickly review or print any form, use the links in the column to the right. Further explanations of all items listed in the column to the right are provided below:

ITEMS TO BE RETURNED TO THE SUMMER PROGRAMS OFFICE - ASAP
- HEALTH EXAMINATION FORM - This form is required; it is a condition of enrollment. It should be filled out by both the parent/guardian (page 1; including the parent/guardian signature in the boxed area) and the physician (page 2; including physician's signature and listing of immunizations).
A copy of the student's medical insurance identification card (both sides) should be attached to the Health Examination Form.
If the student purchases medical insurance coverage from HRH (the school insurance plan), attach a copy of the confirmation (if enrolling on-line) to the Health Examination Form, or attach a copy of the enrollment form if paying by cash or check. The Pratt Health Center will verify all insurance purchases with HRH.
> International students: All international students are REQUIRED to purchase the school's medical insurance plan from HRH. No exceptions.
> For those students who attended Choate's Summer Programs in 2007: We have a Health Examination Form on file from last year. The parent needs to complete page 1 only, being sure to sign within the boxed area (giving permission for treatment during participation in Summer Programs).
> For current Choate students: The parent needs to complete page 1 only, being sure to sign within the boxed area (giving permission for treatment during participation in Summer Programs). Page 2 does not need to be completed by a physician as an updated Health Examination form is currently on file in the Pratt Health Center.
> For incoming Choate students (those enrolling for fall 2008) : The Health Form that was sent in your acceptance packet for the regular academic year should be returned earlier than stated, preferably before June 1st. - MEDICATION DISPENSING AUTHORIZATION - Connecticut State Law and Regulations require a written medication order of an authorized prescriber (physician, dentist, nurse practitioner or physicians assistant), and parent/guardian written authorization for the nurse or camp director to administer medication. Medications must be in an original properly labeled container and dispensed by a physician/pharmacist. Submit if your child is bringing medication from home.
- STATEMENT OF UNDERSTANDING This form is required; it is a condition of enrollment. It is to be signed by both parent/guardian and student stating that, after having read the Student Handbook, they have read, understood and agree to abide by all the rules and expectations of Choate.
- OFF-CAMPUS PERMISSIONS This is for boarding students only to return to our office. With a parent's signature, this gives permission for times when a student is allowed off campus or to leave campus overnight - even to go home. Some permissions are given with the approval of the Dean or Adviser; see Student Handbook for specifics.
Notes Regarding Off-Campus Permissions An introductory note detailing dates students may arrange overnight visits away from campus. - PAYMENT SHEET Print as many copies as needed. This form should accompany payments and will instruct our office on the disbursements of monies that you send to us, whether it be tuition monies, ChoateCard account funds, shuttle transportation fees, SACPass purchase, etc.

- SHUTTLE RESERVATION Complete only if you want to reserve space on our airport shuttles for arrival to or departure from our campus. Reservations may be submitted on-line after your flight arrangements have been made. The Shuttle Reservation Form is also available here to print and submit by mail with payment.
Ground Transportation Resources This is a listing of private limousine services available from our area in the event you wish to make your own travel arrangements or cannot make our airport shuttle times.

SPORTS SELECTION - All boarding students will participate in sports for one hour, four days per week (M-T-Th-F). Day students are encouraged to participate, and if they choose to do so, they must participate for the entire length of their program. Selections may be made on-line, or by printing and submitting the Sports Selection form. All sports are co-educational and recreational (i.e. there will be students with various level of abilities in each group).

These items are to be ordered from and paid for directly to the provider; do NOT make payment to Choate.
- STUDENT MEDICAL INSURANCE - Medical insurance coverage and proof of coverage is required for all students. If a student does not have coverage under a family policy while here on campus, medical insurance must be purchased.
International students are REQUIRED to enroll in the school insurance plan from HRH. No exceptions.
Enrollment and payment (by credit card only: Visa or Mastercard) may be submitted on line directly to Hilb, Rogal, & Hobbs (HRH). Be sure to print the confirmation of your order and submit a copy of the confirmation with the student's Health Examination Form to the Summer Programs Office.
If paying by check or money order, download the Manual Enrollment Summer form (see under Informational Items in the right-hand column and also available on the HRH Web site), and complete it (then attach a copy of the HRH Enrollment Form to the student's Health Examination Form and submit them to the Summer Programs Office). Insurance Brochure - LAUNDRY AND LINEN RENTAL SERVICES - Families have the option of using service from The Campus Laundry by E & R Cleaners (linen rental, laundry, or both) or having the student provide his/her own pillow, blanket, linens/sheets, and/or doing their own laundry on campus. Coin operated machines are available in the Laundry Building; dorms do not have their own laundry facilities. View the E & R letter for more information (see under Informational Items in the right-hand column).
If paying by credit card, the service may be ordered directly from The Campus Laundry via their web site (enter the password SF206), or by calling 1-800-890-7273 (from within the USA) or 603-627-7661 (from outside the USA). Phones are staffed Monday through Friday from 8:00 am - 6:00 pm (EST).

Manchester, NH 03103-9962 - SUMMER PROGRAMS PHOTO - Each summer a panoramic photo (8" x 26") is taken of all students and faculty. The photo is scheduled to be taken July 10, 2008. Families are offered an opportunity to place an order in advance at a discounted price. Orders must be post-marked by June 29, 2008 to receive the discounted price; orders post-marked after June 29, 2008 will pay the regular price. Credit card orders may be mailed, faxed (781.788.4999) or phoned in (781.788.9595) directly to Panfoto; orders paid by check (in US Dollars, drawn on a US bank) may be mailed to:

- OVERVIEW This is an overview of this Web page and denotes which forms are to be returned to our office, which items may be submitted online, which forms are informational, and which items are to be ordered from and payment made directly to the vendor.
- REGISTRATION INFORMATION This lists Registration dates, times, locations, and general instructions.
- STUDENT HANDBOOK The pamphlet is 36 pages long. This will answer most questions that students have about their time here, such as rules (pages 6-10), technology (pages 13-14), what to bring (page 21), what to wear (page 22), residential life (pages 13-20), daily schedules (pages 28-30), etc.

- NOTES REGARDING OFF-CAMPUS PERMISSIONS An introductory note detailing dates students may arrange overnight visits away from campus. Students may take two overnights during their stay with us, with the proper permissions in place.

- TECHNOLOGY INFORMATION Our Information Technology Systems is explained for participants (telephone, e-mail, internet access, computers). Students are encouraged to bring their own computers (either Mac or PC) and printers, if possible.
- COMPUTER USAGE AND SOFTWARE POLICY - All families should review this information if the student is bringing a computer to campus.
- ChoateCard/PHOTO ID The ChoateCard (and photo ID) account is explained. Students may deposit monies into their ChoateCard account so they carry little or no cash on their person while here. The ChoateCard can be used for on-campus purchases or for cash withdrawals. Students will withdraw any balance before departing campus.
- WHAT IS A SACPass? Families may wish to pre-pay for all SAC day trips and activities by purchasing a SACPass. Not available for two-week programs, but any student may purchase each trip individually. Cost: four-week participants - $255; five-week participants - $300.
- SAC ACTIVITIES CALENDAR This calendar shows all the day trips and activities planned by the Student Activities Center (SAC). Note MS (middle school programs) or HS (high school programs) designations. All students will be chaparoned on SAC trips by faculty; middle school students must stay with their chaperone at all times.
- HRH ENROLLMENT FORM Use this form if purchasing student medical insurance by cash or check. Use the HRH on-line ordering system if paying by credit card.

- I-20 IMMIGRATION INSTRUCTIONS For international students who have been issued an I-20 Immigration form (to obtain an F-1 student visa), this notice guides you through the visa application process via SEVIS.
